1.  Complete a copy of the Membership Application in either of the two formats:

• Print a copy of the Membership Packet and fill in the information requested.

• Secured Membership Application.  The password is hsaccc (Make sure you print a copy before you close the form.) Currently secured application is not available.  Please use the Membership Packet above.

2. Payment by either:

• Check  

• PayPal.  Select membership type. When you click Buy Now, you will be directed to PayPal's secure website.  After you complete your transaction, print a copy of your PayPal receipt.
